Chronic Care Management (CCM) is a Medicare-supported program designed for patients living with two or more chronic conditions such as diabetes, hypertension, heart disease, arthritis, COPD, or depression. At Garden State Medical Group, our CCM program provides coordinated, ongoing care between office visits to help you manage your conditions more effectively.

When you enroll in our CCM program, a dedicated care coordinator works with your physician to develop a personalized care plan that addresses your medications, lifestyle goals, preventive screenings, and specialist referrals. This plan is reviewed and updated regularly to reflect changes in your health or treatment needs.

One of the most valuable benefits of the program is 24/7 access to a healthcare professional. Whether you have a question about your medication, are experiencing new symptoms, or need guidance on managing a flare-up, our team is available by phone around the clock. This level of support helps prevent emergency room visits and hospitalizations.

Medicare covers Chronic Care Management services with minimal out-of-pocket costs for most patients. If you have a supplemental insurance plan or Medigap policy, your copay may be fully covered. Our office staff will verify your eligibility and guide you through the enrollment process.
